The lung navigator is responsible to coordinate and work
collaboratively with the radiology, pulmonology, oncology,
radiation oncology, thoracic surgery, and primary care practices to
assure that Lung Cancer Screening and Incidental Pulmonary Nodule
program pathways are implemented across the continuum of care.
Works closely with the care team and entire organization to connect
patients with resources that meet their needs, assists in the
implementation and assessment of effectiveness of screening and
treatment plans including assisting with clinical office duties and
patient care as needed in the department.
DUTIES AND RESPONSIBILITIES:
Serves as patient advocate to ensure continuity of care by
following patients receiving Lung Cancer Screening Program and
Incidental Pulmonary Nodule program services across various care
settings: Inpatient, Outpatient, Emergency, Home Health, health
care providers inter-organizationally, community providers,
regional providers, etc. Provides clinical education and support to
patient and family around medical processes, procedures,
treatments, medications, and management of health and wellness.
Also, provides outreach and education to all health care providers'
inter-organizational, community and regional providers. Assists the
healthcare team in developing annual comprehensive screening and
program plans. Works collaboratively with all community and
regional providers and clinical/office staff in pursuit of patient
satisfaction and quality care. Maintains patient charts in EHR
systems per documentation policies and follows up with patients'
providers outside of the organization. Maintains a working
knowledge of the Center for Medicare and Medicaid Services (CMS)
administrative rules and regulations pertinent to the Lung Cancer
Screening program. Is able to assist provider offices in the
application and implementation of these regulatory pieces. Supports
continuous quality improvement, state and federal regulations, and
health information fundamentals, in pursuit of improved patient
outcomes. Works with software tracking system for patient
identification for screenings and follow-up on incidental pulmonary
nodule findings. Uploads information to American College of
Radiology website to meet Medicare requirements for reimbursement.
Facilitates patient access to community resources as appropriate.
Maintains a good working relationship within the department and
with other departments. Demonstrates the ability to deal with
pressure to meet deadlines, to be accurate, and to handle
constantly changing situations. Demonstrates the ability to deal
with a variety of people, deal with stressful situations, and
handle conflict. Demonstrates the ability to be flexible and
organized. Performs other duties as assigned.
Minimum requirements equal to Certified Medical
Minimum of three years clinical experience involving direct patient
and provider interaction with experience across the continuum of
Data analysis and presentation experience preferred.
