Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 02/23/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Facility: Riverstone Health Position: Case Manager - Home Health Shift: 5x8s days Length: 13 weeks Start: ASAP Must have Home Health exp Must have OASIS charting exp Must have 2 yrs HH experience Must have reliable vehicle and will be reimbursed for mileage The RN Case Manager is responsible for the delivery of quality patient care through the nursing process of assessment, nursing diagnosing, planning, implementation, and evaluation. In tandem with the clinical manager and clinical nursing staff, the RN case manager helps to oversee patient care, nursing case management, clinical documentation, and customer satisfaction with hospice patient care services. The incumbent brings content expertise and knowledge surrounding the program elements, deliverables, and metrics. The work environment is typically fast paced. Work is performed to ensure efficiency of program delivery, incorporating integrity and innovation to ensure a high level of customer service. Ability to travel throughout Yellowstone County.
