Job Description:
TEXT 'Work4ESGW' to to applyApply at: Social Work: Provides
psychosocial support services to hospice patients and their
families; participates in the assessment, planning, intervention,
and evaluation of social and emotional problems faced by the
terminally ill and their families; participates in the development
of the plan of care and prepares progress notes; supports the
Hospice bereavement program, utilizing appropriate community
resources when appropriate; reports major needs of the program to
the supervising MSW and provides input and information into
planning and evaluation of Hospice program. The Social Worker
provides services under the direction of a supervising social
worker. Supports applicable QAPI/QA studies.Volunteer Coordination:
Recruit and train volunteers. Maintain and update volunteer
training program. Coordinate hospice volunteer program and new
program development. Meet with new patients and/or caregivers at
admission to offer volunteer services. Continue to offer
opportunities to change volunteer plans of care on a monthly basis.
Evaluate volunteers at least on an annual basis. Plan and execute
volunteer in-service training and recognition events. Interface
with outside organizations to create long-term sources of volunteer
services. Document volunteer recruitment, training, services, and
evaluations in compliance with CMS requirements and industry best
practices.Spiritual Care Coordination: Meet with new patients
and/or caregivers at the time of admission to assess spiritual
needs and create a spiritual plan of care, considering all input
from interdisciplinary team. Offer opportunities to update POC.
Provide guidance to the IDT regarding spiritual assessment,
observation, and expressed needs of the patient/family. Conduct
in-services for staff of all disciplines regarding
interdisciplinary spiritual care assessment, intervention,
documentation, and the role of the spiritual counselor on the team.
Consult with team members when community clergy is directly
involved with the patient/family. Consult with, and serves as a
resource to community clergy directly involved with the
patient/family. Provide spiritual care support for hospice
staff.General Team Responsibilities: Provide backup for IDT
documentation and general operation of business. Facilitate
efficient communication among staff members. Participate in care
for facilities and vehicles. Participate in daily huddle, staff
meetings, IDT meetings, and safety and general
training.RequirementsMust have knowledge of the application of
crisis intervention skills.Must have the ability to respond calmly
and effectively in stressful situations; assess patient needs;
reassess continuing patient needs; communicate effectively with
other health care providers.Strong, effective written and verbal
communication and interpersonal skills.Must possess strong
organizational and time management skills.Physical
RequirementsAbility to move independently or with reasonable

physically and mentally able to perform all requirements of the
job.Current driver's license and reliable transportation.All

Social Work from a social work curriculum accredited by the Council
on Social Work Education with proper supervision.Two to three years
of experience working with older populations, patients with chronic
